After we receive your email, we will then contact you and send you a code to download our clinic documents. Once we have received all your child's completed clinic documents, we will contact you to schedule an assessment.
Payment Information
Achieve & Thrive Occupational Therapy Services is a private pay provider. As such, we do not accept any insurance. However, that does not mean your insurance company won't cover our services.
